Faktor Risiko Prediabetes: Isolated Impaired Fasting Glucose (i-IFG), Isolated Impaired Glucose Tolerance (i-IGT) dan Kombinasi IFG-IGT (Analisis Lanjut Riskesdas 2013)

Abstract
An increasing trend of prediabetes is observed worldwide due to lifestyle changes. Several determinants
and metabolic abnormalities have been associated with the condition, but deeper analysis regarding
their causal relationship to exclusive criteria of prediabetes, namely isolated impaired fasting glucose
(i-IFG), isolated impaired glucose tolerance (i-IGT) and mixed IFG-IGT has not been done yet on
nationwide data, particularly in Indonesia. This analysis using data extracted from the 2013 Basic
Health Research (Riskesdas) was performed to study associations among factors contributing to
prediabetes. Bivariate and multivariate relationships were tested of i-IFG was found to be 20.4%,
15.6% for i-IGT and 12.4% for mixed IFG-IGT. Substantial risk factors for i-IGT include: age ≥30
years, female gender and fatty food consumption ≥1x/day. While determinants for i-IFG were age
group ≥40 years, sweet food consumption >1x/day, and drinking coffee ≥1x/day. As for the mixed
IFG-IGT category, age group ≥45 years and obesity were found to be potential risk factors. Hence,
prediabetes and associated factors become serious public health problems and require effective
intervention related to age, gender and dietary risk behavior to minimize further negative impacts
attributable to metabolic diseases.

Abstrak
Prediabetes semakin sering dijumpai di seluruh dunia seiring meningkatnya kejadian diabetes mellitus
akibat perubahan gaya hidup. Beberapa determinan dan gangguan metabolik telah dihubungkan
dengan keadaan tersebut, tetapi analisis mendalam terkait hubungan kausal faktor tersebut terhadap
prediabetes dengan kriteria eksklusif seperti isolated impaired fasting glucose (i-IFG), isolated
impaired glucose tolerance (i-IGT) dan campuran IFG-IGT belum dilakukan untuk data skala
nasional khususnya di Indonesia. Analisis yang menggunakan data sekunder Riskesdas 2013 ini
dilakukan untuk mengkaji hubungan determinan yang diduga berperan pada timbulnya prediabetes.
Data diolah melalui uji bivariat dan multivariat. Secara umum, proporsi i-IFG dijumpai sebesar 20,4%,
i-IGT sebesar 15,6% dan campuran IFG-IGT sebesar 12,4%. Hasil analisis multivariat menemukan
bahwa faktor risiko bermakna i-IGT meliputi usia ≥30 tahun, jenis kelamin perempuan dan konsumsi
makanan berlemak ≥1x/hari. Sementara determinan i-IFG adalah usia ≥40 tahun, konsumsi makanan
manis ≥1 x/hari, minum kopi ≥1 x/hari. Pada kombinasi i-IFG dan i-IGT, faktor risiko potensial yang
dijumpai adalah kelompok usia 45 tahun ke atas dan obesitas. Karena itu, prediabetes dan faktor
risiko terkait menjadi masalah kesehatan masyarakat yang memerlukan intervensi tepat dengan
fokus pada faktor gender, umur dan perilaku makan, untuk meminimalkan dampak negatif lebih
lanjut akibat penyakit metabolik.
